Once the parent and student have read the regulations, they must sign and date this form and turn it in to the High School office.
The High School Athletic Code of Conduct is in effect from the first day a student joins a high school athletic team. The Code remains in effect 24 hours a day, 365 days a year (regardless of time of day, week, month, or year, location of events / place of events, and/or particular sport season)until completion of a student’s participation in high school athletics .
